- What is happening. The symptom as you see it, and when it started.
- What is already in place. Roughly what the environment looks like, even if the documentation is out of date.
- What outcome you need. A fix, a second opinion, a build, or a plan you can take to a budget holder.
- Timescale. Whether this is urgent, a planned project, or something you are scoping for later in the year.
